What to do in case of an insured accident:
Call accredited doctor / "Universalna"contact center (0 800 500 381) and follow their instructions;
Tell an operator/accredited doctor the following data:
Name of the insured person;
The individual service card and policy numbers;
The accident circumstances description and nature of the necessary aid;
Location and contact phone number
Other information the insurer may need.
Documents necessary for insurance compensation:
Recipient's (the aggrieved party receiving the insurance indemnity) personal documents copies:
Passport (pp.1, 2, and 11);
Certificate of grant the ID number;
Insurance certificate;
Claim and application forms:
An insurance indemnity claim
A non-cash payment claim
Competent bodies original documents:
Medical institution standard certificate containing information on diagnosis and aid rendered, attested by the institution stamp and signed by the doctor in charge of the case;
Sick-list complying with laws of Ukraine;
Other documents according to the policy conditions
